When AI Writes the Code, Who's Accountable When It Breaks?

Last quarter, a team I know shipped a bug in production. Customer data was briefly exposed to wrong users — not stolen, but visible. The root cause was a race condition in code that had been written almost entirely by an AI assistant. The post-mortem was awkward. The developer who "wrote" the code had accepted a suggestion from Copilot without deeply reviewing the concurrency logic. The engineering manager wanted to know who was accountable. The developer said "the AI suggested it." The manager said "you accepted it." Both were right. Neither answer helped anyone. The accountability gap is real Most AI coding tool discussions focus on productivity and output quality. The accountability question gets skipped because it's uncomfortable and because it doesn't have a clean answer yet. But it's coming. As AI-generated code moves into more critical systems — billing, auth, healthcare, finance — the question of "who's responsible when it breaks" will matter more, not less. Here's how I think
