However, from January 2026, the system has changed completely. Now, the process is divided between two platforms-Intealth (for ECFMG certification) and FSMB (for exam registration). What Has Changed in USMLE 2026? The biggest update is that ECFMG no longer handles USMLE exam registration. Instead, the Federation of State Medical Boards (FSMB) now manages exam applications, scheduling, and related services for international medical graduates (IMGs).
