That’s where governance drift shows up. Not as a visible failure, but as a gradual separation between what was defined and what is actually happening. The issue isn’t the absence of rules. It’s the absence of enforcement at execution. Failure isn’t the moment something breaks. It’s the accumulation that made the break inevitable.
